On June 25, 2009, the Portland Trail Blazers selected Mills with the 55th overall pick making him the first Saint Mary’s player drafted since 1983 and the highest pick since 1961.

Because of the 2011 NBA lockout Mills returned to Australia to play in the National Basketball League (NBL). He reportedly turned down significant offers from various European clubs in order to sign with the Melbourne Tigers on August 29, 2011.

On March 27, 2012, Mills signed a contract with the San Antonio Spurs. Mills recorded his first NBA double-double with 34 points and 12 assists in a 107-101 victory over the Golden State Warriors on April 26, 2012. Mills surpassed Andrew Bogut’s 32-point performance in January 2010 to set the record for the most points scored by an Australian in a single game in the NBA.

Mills re-signed with the Spurs on July 13, 2012. He scored a season-high 23 points in the Spurs’ final regular-season game against Golden State on April 15, 2013. The Spurs advanced to the 2013 NBA Finals, where they were defeated in seven games by the Miami Heat. Mills missed the final four games of the NBA Finals because of the removal of an abscess on his right foot.

On December 29, 2020 he became the tenth player in Spurs history to play in 600 games. Mills is the Spurs’ longest-tenured player as of December 2020 and the only player remaining on the roster who was a member of the Spurs’ NBA championship-winning team in 2014.
